2024 Vision Service Plan (VSP) Summary

Use the resources menu to the left to access useful information such as your benefit summary, links to find a vision provider near you, enrollment forms, and many other important documents about your vision plan.

Please refer to your Summary Plan Description or call customer service for specific benefit details, limitations, and/or exclusions.

Find a Vision Provider

Please use these instructions to find a Vision Provider in your area.

Visit Find a Vision Provider.

• If you already have a member log in, Sign In to your account.
• If you are not a member and want to create an account, follow the instructions under the link “Register Now.” Select the Member/Employee ID button to continue.
• To view VSP providers without logging in, select the Find a VSP Doctor link at the top of the page and then select the link “Not a Member?.”
• Enter your State and Zip Code.
• Select the additional criteria that will streamline your search.
• Click Submit.

A list of Vision Providers will display for your review.

Out-of-Network Reimbursement

If you’ve received eye care services from an out-of-network provider, you must submit a claim to request reimbursement. Log on to vsp.com and select the Claims & Reimbursement link to complete your reimbursement request online. To expedite the processing of your claim, have your itemized receipts or statements ready to submit with your request.

Submitting A Claim Online:

• Be sure your receipts have been scanned and are accessible by your computer.
• Login to your vsp.com account and access the Benefits and Claims section
• Click on "Start New Claim"
• Complete the fields and follow the prompts
• Upload your receipts

Submitting A Claim by Mail:

• You can also send your receipts by mail.
• Access the Benefits & Claims section on your account at vsp.com.
  • Click on “Start New Claim”.
  • Complete the fields and follow the prompts.
  • After completing the claim form, you may print and mail copies of your claim form and receipt(s) to:
