Employees have three opportunities to enroll in AFLAC policies:

• Within 60 days of hire
• During a qualifying life event (Marriage, Divorce, Birth, Adoption); or
• Each year during Open Enrollment (November 1st - November 30th).

Policy options include:

• Accident
• Critical Illness
• Hospital Indemnity, and
• Short Term Disability.

To Enroll

• Visit myapplications.microsoft.com - If you are not on a computer connected to the COH network, you will be asked to log in with your City E-Mail address, network password, and a multi-factor authentication code.
• Select Paylogix App
• Click on the specific AFLAC policy that you are inquiring about in order to obtain more information.  
• Choose the enroll option at the bottom of the page.
