Long Term Disability
Long‐Term Disability is designed to replace a portion of your monthly salary in the event of a covered disability. To prevent over‐insurance, benefit payments are reduced by deductible sources of income, such as state disability and unemployment income. Benefit calculations are based off your regular weekly rate of pay, not including commissions, bonuses, over‐time, or other compensation. Long Term Disability (LTD) Benefits

Long Term Disability benefits will pay 60% of the employee's monthly salary up to a maximum monthly benefit of $10,000.

Benefits will be paid for the duration of the disability. Benefit reductions will occur at age 65.

To file a cliam for LTD benefits, you will need to submit claim forms completed by you: (Employee Claim Form), your physician: (Physician Claim Form), and your employer: (Employer Claim Form).

Long Term Disability benefits are paid on a monthly basis.

Checks will be sent to your home address.

180 day elimination period before Long Term Disability benefits begin.

Your benefit will be reduced by amounts you receive from Social Security, an employer sponsored retirement plan and other group disability benefits.

A pre-existing condition is a condition for which you received medical treatment, consultation, care or services including diagnostic measures, or took prescribed drugs or medicines in the 3 months just prior to your effective date. Pre-existing conditions are excluded from coverage if a disability due to that condition begins in the first 12 months after your effective date of coverage.

Disabilities primarily based on self-reported symptoms or disabilities due to mental illness have a limited pay period of up to 24 months unless you remain hospital confined at the end of the 24-month period.
