You can access the PERS Survivor Beneficiary Designation form here, or directly on the Public Employees Retirement System (PERS) website.

Some additional information about your retirement benefits:

Current Employer-Paid contributions are 44.00% for Public Safety and 29.75% for Non-Public Safety employees. 

Effective July 1, 2023, the PERS contribution rates will be increasing to 50.00% for Public Safety and 33.50% for Non-Public Safety. 

Visit the PERS website to view information such as:

  • Access the PERS Summary Plan Description for Regular and Police/Fire members. Be sure to select the correct summary plan based on your hire date/enrollment in PERS;
  • Create your account (see website instructions link on the left) and view your personal information, calculate retirement benefits and get an estimate to purchase additional service credit. Note: newly hired members should wait 90 days before setting up an account;
  • Get access to PERS forms such as PERS Survivor Beneficiary and Change of Personal Information forms;
  • Find out when the next PERS sponsored informational presentation will be held for programs such as Planning Ahead for Your Retirement;
  • View the on demand Audio Programs for details and information regarding your benefit;
